Before you appoint a contractor, check their health and safety record. RIDDOR reports are not published anywhere, so the only way to see them is to ask. This page gives you a signed declaration to send your contractor, and quick links to HSE's public registers.
HSE publishes enforcement notices (kept for 5 years) and prosecutions of organisations (kept for 10 years). Notices appear about 5 weeks after they're served; convictions about 9 weeks after. Councils keep separate registers for the premises they inspect.

The questions follow the SSIP core criteria that accreditation schemes use: RIDDOR figures for the last 3 years, the last two accidents or incidents, and any enforcement or prosecutions in the last 5 years.
Paste one contractor per line as company name,date of their last declaration (YYYY-MM-DD; leave the date blank if you have none). We sort them so the ones to chase come first.
How often to renew is your decision: no law sets a period. CDM 2015 asks for "reasonable steps".
